This woman in her mid-twenties from north of Boston came to my office to discuss breast augmentation. As with all of my consultations, we spent a great deal of time defining and understanding her aesthetic goals from surgery. We discussed the concept of fill volume, which is the volume of breast implant that will fill the breast tissue to the point of breast fullness without roundness in the upper breast. This patient wished to have some roundness in the upper breast so she chose an implant larger than her fill volume.
Her breast augmentation was performed at my practice through an inframammary (below the breast) incision placing saline implants below the pectoralis muscle. Her implants are 400cc; I estimated her fill volume to be 300cc. I do not use drains or tight chest wraps after breast augmentation surgery and patients take ibuprofen for control of pain and swelling. Most patients return to routine activities and work within a few days of surgery.
